Diagnosis

As a counselor, we will have to determine a diagnosis due to billing insurance for the services rendered.  Some diagnoses may not be covered by insurance and the reimbursement for therapy.  We will try to provide an accurate diagnosis in the beginning, by utilizing assessment tools and information gained from intake, but be aware a diagnosis may change during your counseling process, and will be reviewed with goals possibly changing or more information is gained.

Confidentiality

Client’s clinical record is maintained confidential unless the client chooses to sign a release of information and then that will be discussed with the client as to who to release the information to and what information they want released.  There are times that the therapist may need to break confidentiality in the event a disclosure of harm to self or others, someone is putting the client or anyone else in danger, any illegal behaviors, or if subpoenaed to court.
